Yea, your graphics card is faulty. Those are the signs. :/

If it was the power supply, the system would shutdown when you play a game.
    Here you are at idle.

Does it happen every time? Sometimes I get something similar at the very top of my monitor. I switch the monitor off and on again and it goes away.

Just had the same thing on my graphics card, a GTX560ti, had no picture when my pc was plugged into my AV system, so i plugged it straight into a TV and had the same type of screen artifacts and lines as you.

I tried the DVI port on my graphics card and it works fine, turns out the HDMI port was knackered, its the second 560ti Ive had that has developed a problem with the mini HDMI port

Update - received a refurbished card from EVGA - put it in my system and there was no green lines (YAY), but idling at 65 deg :(

I then loaded a game (Civ V) and it decided to crash my system, with the fans on the card blowing hard.

guess time to talk to EVGA again

on start it reads 67 degrees... doing nothing evga reads 38 - 40 (watched for 1 min)

refreshing my windows experience (only thing I thought of) it goes straight up to 102 degrees and the system restarts
