That comes down to insufficient testing in multiple environments

Well, my first laptop (6 years ago) suffered from overheating after a while. I cracked that baby open and found that a thick thermal pad was used between the CPU and heatsink assembly. The ~1mm thick pad had become brittle over the years and was deformed to the point where much of the CPU and the heatsink weren't in contact with it at all.

Most laptops are. Sharp corners, wrong plastics and wrong power plugs for the stresses that they will be subject to. Screen hinges crack and screen frames warp as you open and close them, because again the stresses of this action have not been considered.

Apple Macbooks are a shining example of how it is done right.

There were a number of heat-sinks around some years back where fitting by ham-fisted users caused them to crack the CPU core; not really a serious design fault but it spawned a sub-industry in shims and AMD fitted corner pads on later CPUs to aid proper fitting.

There was a sticky in Hardware listing the worst offenders, 'heatsink cracked core' will google up lots of info.

Intel's desperate push to compete against the new Barton-core Athlons, resulting in some strange results, such as the obscenely expensive "Extreme Edition" processors based on the fireball of a Prescott core.

What about bridge collapses? The millennium bridge in London springs to mind, as does the bridge in the US that opened 50-60 years ago (not sure what it's called, but it always gets shown on any programmes related to bridges)
    EDIT: This one -

Not sure where it is though.
