isn't that a bit extreme for a game, i thought that was for problems with windows. i've uninstalled and reinstalled and put my player folder back and doesn't work so will try without my player folder and will have to start from scratch.

No, not formatting and reinstalling, I mean Windows System Restore.
Type System Restore in the start menu, and restore from a point from yesterday (or whenever you know it was working).

Have you had any windows updates in the last couple days, or updated any drivers, or even changed any settings (default sound device or something?)

Sounds silly, but try plugging in a microphone (even headphones and force windows to detect them as a mic - speakers are just mics wired in reverse, essentially)...
costs nothing, takes 10 seconds, and has helped me with the same issue (albeit no windows updates or anything like that) in COD4 Multiplayer and COD5 single- and multiplayer.

This always works perfectly for me. I've had this problem since COD2. You think they'd have found the problem by now, but I guess there all too busy trying to remove everything that made the game fun...

Oh.. I had to fix this on Vista, I hope its not a different problem. Soultion:
    1:Go to control panel
    2:Open Hardware and Sound
    3:Open Sound
    4:Go to Recording tab.

5: Right click in the box showing devices and tick show disabled devices.

6:Enable Stereo Mix by right clicking it and selecting enable.

    7:play COD!

    Hope it works.
