I am looking for an app that will turn my mac mini into a home phone.
What I want to do is use my bluetooth headset and the mac as the phone so to say.
I don't want skype or anything I already have phone service I just need to the mac to be the phone.

Some modems will let you use them as a phone, you'll have to have a quick google to find out the details though.

if you have a cell phone with bluetooth (and bluetooth in your mac) you can pair your cellphone with address book and that should do what your looking for. once the phone is paired you can find the "card" of the person you want to cal and click the label next to the number (home, work, mobile) and then just click "Dial with YOUR_DEVICE_NAME_HERE" and it will dial that number on your cell phone.

or you could try using something like Asterisk if your feeling adventurous
