I respected you for the Crysis videos, but I love you for that one name Gunsmith! That game was amazing.
Oh, another one: Duke Nukem 3D. Technically I never completed it as I used cheats to get to the next level. The puzzles were just too damn hard at the time! Maybe I'm just an idiot, but there were quite a few levels I just ended up looking around for up to an hour before giving up in frustration.
Same, although I never actually finished it. It's my honourable mention. I got to (what I later learned was) the second-to-last boss and other things got in the way, and I never got back to it. Still a brilliant game, though, I plan to revisit it once I have more time (and space, for the damn console). edit - You are not human.
Demon's Souls....hardest game I ever played the first time through. The game really forced you to be creative and learn its fighting mechanics. It also doesn't hold your hand and is very unapologetic

Ninja Gaiden is definitively one of the most difficult games to ever have existed. The STALKER series is quite fun and difficult although half of it is based on the lack of any direction, and the anemia you have.
You'll need it. End boss is a bitch, but who you face depends on your clan and the moral choices you choose in.. ... the game.
In Call of duty modern warfare, that bit at the big wheel funfair bit, on veteran. That took so, so many attempts.
Too right they were. Super Mario Bros 3 on the NES? Geez, that was tough. Well I nearly, nearly finished the game, but never actually finished the final level ... ... until I discovered emulators and the ability to save at any point you like. This, I think, is key.
Did you hide round the side of the dodgem cars? Or stay on the grassy mound with Captain MacMillan? The dodgems make it much, much easier.
a newer game that's hard and doesn't hold your hand all the way through? go play mount & blade: warband, on the hardest settings. or if you're sadistic, play the mod blood & steel as the peasant revolutionary class on mommy If u know what I mean?
Dwarf Fortress perhaps its more a case of steepest learning curve than hardest (no win condition you see) but its still a head scratcher. And great fun for it.
